Who We Serve

Our hospice welcomes individuals facing a life-limiting illness, their families, and caregivers. Regardless of age, background, or financial situation, our services are available to anyone in need within Sarnia Lambton and surrounding areas.

Referral Process

Community and Supportive Care Referral Process:

Accessing our grief and bereavement services is straightforward. Referrals can be made by:

  • Healthcare professionals (e.g., doctors, nurses, or social workers).
  • Family members or friends of the individual.
  • The individual themselves.

Hospice Residence Referral Process:

Accessing hospice care is straightforward. Referrals can be made by:

  • Service referral from Ontario Health atHome (individual can self-refer to Ontario Health atHome)
